The Farmacy connects local physicians with surplus healthy produce from both seasonal producers and grocery store retailers, ensuring that primary care providers have on-hand resources to provide to Medicaid patients that are suffering from or at-risk of nutrition related illnesses. This "point-of-care" resource allows individuals and families convenient access to healthy food paired with diet and nutrition recommendations from their physicians. Data collected from our physician partners indicates that this results in improved health-outcomes for their patients and more frequent/consistent return visits for care. We are currently piloting this program at Matthews-Vu Medical Group and Sunrise Health Care, PC.
